Full-Pipeline Random Bit Generator

Top-level random bit generator controller implementing the SP 800-90C construction. Manages the full entropy pipeline: quantum-ratio-gated startup sequence, output gated on both health AND quantum checks, and selectable DRBG output stage (SHA3 Hash_DRBG or DRBGVS-tied AES CTR_DRBG).
